This 1861 Navy revolver barrel exploded because a chamber was loaded without powder (ou the powder failed to ignite), and the second chamber was fired on the bullet stuck midway in the bore. The shooter was using 3 Fg Swiss powder and a full load. Thanks to the safe Colt design and the good material of the repro no one was injured.
The barrel split, where the material was the weakest: where the front sight and the loading lever catch is milled in the the bore.
